Create a new canvas with this size: 400x100

create yourself a bg like this



Follow these steps:

Filter > Blur > Radial Blur

Filter > Distort Glass

Filter > Texture > Cracquelure

Press CTRL U, make sure you have these settings from top 360, 0, -23

write your username or whatever you want on there and this is what i came up with:
